BILL NUMBER:S4147
Title of bill
An act to amend chapter 633 of the laws of 2006 amending the public health law relating to the home based primary care for the elderly demonstration project, in relation to the effectiveness thereof
Purpose
The purpose of this bill is to continue access to home based medical care for the elderly living in the community as part of a demonstration program.
Summary of provisions
This bill would extend for five (5) years the demonstration program that currently allows certain nursing homes that also provide a variety of community based care to provide home based physician, nurse practitioner and physician assistant services to elderly patients.
Justification
Some of New York State's most experienced geriatricians work in nursing homes or for long term care providers that operate a continuum of programs both residential and in the community including nursing homes, residential communities and community based programs for the elderly. Currently, the state is actively encouraging nursing homes to down size and further push toward community based care. And, many nursing homes have responded by reshaping the care delivery system to try and meet seniors' needs in their community rather than in the nursing home.
This legislation extends the authority for nursing homes to operate a physician practice to serve community based seniors in their homes. Allowing a limited number of nursing homes to test this service delivery model will allow the state to judge the impact of borne based medical care on the condition of the elderly and disabled. In many cases, home based services, largely paid for by Medicaid could forestall placement of people into more expensive long-term care services, such as nursing homes, where the state ends up paying a substantial part of the cost of care through Medicaid.
