AN ACT to amend the public health law, in relation to the creation of an online standardized medical physical examination form
The People of the State of New York, represented in Senate and Assembly, do enact as follows:
Section 1. The public health law is amended by adding a new section 25 to read as follows:
§ 25. Standardized medical physical examination form. The commissioner shall establish an online standardized medical physical examination form, which shall be utilized by, but not limited to, schools, colleges, universities, camps for children and doctors' offices.
§ 2. This act shall take effect immediately.
